SUMMARY:
Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for individuals served.

Successful completion of Background, MVR and Drug Free Workplace Screenings.
EDUCATION and/or EXPERIENCE:
Bachelor's degree from an accredited four-year college or university with emphasis in the areas of psychology, social work, health education or related human services field. Preferred one year of full time or equivalent experience working with adults who have serious mental illness and children who have serious emotional disturbances. - OR-
Have a minimum of a bachelor's degree from an accredited university and at least three (3) years full time experience working with children who have serious emotional disturbances or two (2) years full time experience in working with adults who have serious mental illness.
CERTIFICATES, LICENSES, REGISTRATIONS:
Certification or certification-eligible by Medicaid as a Case Manager.
SPECIAL SKILLS:
Experience in providing case management and advocacy services to individuals in an outpatient mental health setting. Excellent written and verbal skills necessary. Computer literate; trained in electronic health record system upon hire.
