Case Manager - Forensics

SUMMARY:

Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for individuals served.

Pay:$19.25/hr

The Forensic Case Manager is responsible for monitoring and assisting individuals involved in the criminal justice system who exhibit serious impairment due to mental illness. The focus of the position is on serving individuals under court supervision adjudicated Incompetent to Proceed or Not Guilty by Reason of Insanity consistent with Florida Statute 3.212 and Rules of Criminal Procedure 916. The Forensic Case Managers goal is to enhance the independence, autonomy and quality of life of the individual served, while ensuring compliance with all judicial requirements to ensure public safety.

Forensic Services:

  • Monitors mentally ill individuals transitioning from commitment to the department pursuant with Chapter 916, F.S. Monitors the progress and advises the court and the attorneys of treatment progress for appropriate felony offenders on conditional release.
  • Maintains close working relationships with the judicial assistants in the circuit, the Office of the Public Defender, the Office of the State Attorney and the Clerk of the Courts Office.
  • Assists as a resource person to the community case managers, state hospital staff, SAMH staff and jail/correctional personnel, on an as needed basis in matters involving forensic persons.
  • Completes regular risk assessments and monitors compliance with the conditional release plan to ensure public safety.
  • Prepares written reports for the court, including placement plans, progress reports, violation reports and other correspondence.
  • Provides competency restoration services as required.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Electronically documents case management activities according to Agency, State, Medicaid and other accreditation standards and policy and procedures.
  • Provides program linkages with other agency programs and community resources to coordinate service delivery, remove impediments to effective care, and achieve established objectives.
  • Advocates for and links the individual with identified needed services and resources. Coordinates all services ensuring individual's needs are being met toward completion of service plan goals.
  • Works with the individual's family and natural support system in the individual's natural environment.
  • Monitors and ensures compliance in service delivery and documentation.
  • Participates as a team member and contributes to the enhancement of the department and agency.
  • Follow SPBH Policy No. CM-3 on Forensic Case Management
  • Complete agency registrations for Phoenix individuals within three business days of their arrival to ALF .
  •  Complete SSI/SSDI, OSS, and Community Living Plan within 7 days of enrolling the individual
    into TCM services.
  •  Coordinate with Dayspring Village as outlined in the DSV MOU.
  •  Participate in the weekly Phoenix meeting and monthly treatment team meeting.
  • Attend medication management appointments with assigned individuals.
  •  Monitor assigned individual’s conditional release plans.
  •  Create court reports and attend court hearings for assigned individuals.
  •  Coordinate competency restoration services for assigned individuals.
  • Coordinate discharges for assigned individuals who have been identified for discharge planning.
  • Complete the Florida Forensic Specialist training within one year of employment

QUALIFICATIONS:

Successful completion of Background, MVR and Federal Drug Free Workplace Screenings.

EDUCATION AND EXPERIENCE:

Bachelor's degree from an accredited four-year college or university with emphasis in the areas of psychology, social work, health education or related human services field.  Preferred one year of full time or equivalent experience working with adults who have serious mental illness and children who have serious emotional disturbances - OR-

Have a minimum of a bachelor's degree from an accredited university and at least three (3) years full time experience working with children who have serious emotional disturbances or two (2) years full time experience in working with adults who have serious mental illness.

Experience in providing case management and advocacy services to individuals in an outpatient mental health setting. Excellent written and verbal skills necessary. Computer literate; trained in electronic health record system upon hire.

Certificates, Licenses, Registrations:
Certification or certification-eligible by Medicaid as a Case Manager.
