Provide community-based, recovery-focused care to adults with severe and persistent mental illness. As part of a multidisciplinary FACT-IL team, you’ll deliver critical treatment, rehabilitation, and support services that promote independence, stability, and long-term recovery.

What You’ll Do
• Provide case management, rehabilitation, and support services in community-based settings
• Conduct mental health status assessments and assist with biopsychosocial assessments
• Develop, implement, and update individualized recovery and service plans
• Coordinate care and connect clients to community resources and supports
• Advocate for client needs including housing, medical, legal, and social services
• Assist clients with activities of daily living, transportation, and life skills development
• Provide education on mental health conditions, symptoms, and medications
• Collaborate with families, caregivers, and interdisciplinary team members
• Maintain accurate, timely, and compliant clinical documentation
• Ensure adherence to DCF guidelines and FACT program standards
What You Bring
• Bachelor’s degree in a behavioral science field
• Experience working with adults with severe and persistent mental illness (e.g., schizophrenia, bipolar disorder, personality disorders)
• Strong communication skills and ability to work with diverse populations
• Ability to make independent decisions, including responding to crises and emergencies
Preferred
• Targeted Case Management (TCM) Certification
• Experience in community-based behavioral health or FACT programs
• Familiarity with Microsoft Office and electronic health record (EHR) systems

Additional Requirements
• Successful completion of Level II Background, MVR, and Drug-Free Workplace screenings
• Ability to work independently and in community-based environments
• Completion of all required trainings within designated timeframes
• Strong attention to detail and compliance with documentation standards

• Occasional lifting up to 25 pounds
