What You'll Do
- Conduct nursing assessments, document services, and participate in daily team meetings
- Provide medication management, including administration of injections
- Coordinate medical and psychiatric services within the community
- Collaborate with families, community agencies, and providers to support care plans
- Arrange and track lab work, prior authorizations, and medication delivery
- Manage medications and samples to ensure continuity of care
- Schedule psychiatric appointments and coordinate referrals
- Deliver nursing services in clinic settings
- Maintain accurate documentation and ensure compliance with all regulations
- Participate in Quality Improvement initiatives and required trainings
- Uphold a respectful, team-oriented, and client-centered approach to care
What You Bring
- Graduation from an accredited school of Practical Nursing
- Active Licensed Practical Nurse (LPN) license in the State of Florida
- Valid Florida driver's license, acceptable driving record, and proof of insurance (if applicable)
- Ability to pass Level II Background, MVR, and Drug-Free Workplace screenings
Preferred
- Experience working with individuals with severe and persistent mental illness
- Experience in community-based or behavioral health settings
- Strong organizational, communication, and coordination skills
- Ability to work independently and as part of a multidisciplinary team

Additional Requirements
- Completion of CPR/First Aid and HIV/AIDS training within first 6 months (if not already certified)

- Maintain confidentiality and professionalism at all times
Work Environment & Physical Requirements
- Clinic-based work
- Regular standing, sitting, walking, and computer use
- Occasional lifting of up to 10 pounds
- Moderate noise level in both office and community settings
- May involve travel within the community to support client care
