Support individuals in a Psychosocial Rehabilitation (PSR) / Day Treatment program by delivering services that build life skills, promote independence, and improve overall functioning. Your work will help individuals strengthen daily living skills and succeed in their communities.

• Assist with daily operations of the Psychosocial Rehabilitation (PSR) program
• Conduct registrations, assessments, and documentation in the electronic health record (EHR)
• Develop and implement individualized treatment plans and monitor progress
• Facilitate group sessions focused on social, educational, and life skills development
• Maintain accurate progress notes and attendance records
• Collaborate with psychiatrists, nurse practitioners, case managers, and care teams
• Provide education to clients and families regarding health and medications
• Ensure client safety, confidentiality, and compliance with all standards
• Support program quality and participate in team meetings and initiatives
• Bachelor’s degree in Counseling, Social Work, Psychology, or related human services field
• Experience in behavioral health, PSR, or day treatment settings
• Strong group facilitation and engagement skills
• Knowledge of treatment planning and documentation standards
• Effective communication and teamwork abilities
• Consistent, high-quality group facilitation and client engagement
• Accurate, timely documentation aligned with treatment goals
• Positive client progress in activities of daily living (ADLs)
• Strong collaboration with multidisciplinary teams
• Compliance with program standards and regulatory requirements
• Successful completion of Level II Background, MVR, and Drug-Free Workplace screenings
• Ability to work in both facility and occasional community-based settings
• Commitment to professionalism, teamwork, and ethical standards
