SUMMARY

Providing direct service and supervision to individuals in our PSR / Day Treatment program - including educational activities, rehabilitation, and care services designed to develop, maintain, or restore the skills necessary to allow individuals served to improve their Activities of Daily Living (ADL) skills and to function successfully in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Assist with daily operations of the Psychosocial Rehabilitation Program.
  • Assist Supervisor with monitoring program operations to ensure compliance with applicable internal and external standards and regulations.
  • Performs registrations and assessments as required and appropriate via electronic health record. Assesses each client using Quality Indicators and documents in electronic health record.
  • Establishes treatment plans electronically for individuals served and develops methods for individuals served to reach goals.
  • Ensures that progress notes are entered electronically and reflect progress relative to treatment plan goals.
  • Facilitates groups designed to meet social, educational, skill building, and therapeutic needs of the individuals served utilizing evidence-based practices.
  • Ensures the safety and confidentiality of individuals served.
  • Maintains current client attendance lists.
  • Communicates with psychiatrists, nurse practitioner, case managers, mental health staff about the individuals served.
  • Provides patient/guardian health and medication education.
  • Participates as a team member and contributes to the enhancement of the department and agency.
  • Performs other duties as assigned/required.
