EDUCATION and/or EXPERIENCE

This Licensed Practical Nurse position requires graduation from an accredited school of Practical Nursing and a valid Florida Nursing License.

Experience working with individuals with substance use issues is preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Must be licensed as an LPN in the State of Florida.

Possession of a valid Florida driver's license, acceptable driving record, and proof of personal automobile insurance if the position requires. Complete State of Florida mandatory level 2 background screening prior to start of employment if direct contact with children/adolescents, vulnerable adults, or the developmentally disabled may be involved. Complete Mandatory training, CPR/First Aid and HIV/AIDS training during the first six (6) months of employment and updated if required.

SUMMARY

Performance of selected acts, as defined by the Nurse Practice Act in Florida, including the administration of treatments and medications, promotion of health and wellness, and prevention of illness under the direction of a registered nurse or physician. Participates in Quality Improvement activities and attends medical management staff meetings and mandatory staff training sessions.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Administers medications, including injections, according to prescribed orders.
  • Obtains Prior Authorizations (PA) through insurance companies in a timely manner to ensure medical continuity of care.
  • Collects vital signs and other health screening information and documents in EMR system.
  • Assists provider with documentation of treatment plan.
  • Assists with prescription refills as necessary.
  • Assists with reconciling medication list, entering pharmacy information, reviewing allergies, and verifying medication consents have been completed.
  • Assists with incidentals and/or patient assistance applications for medication/treatment.
  • Participates as a team member and contributes to the enhancement of the department and the agency.
  • Coordinates lab work and collects specimens for diagnostic testing.
  • Coordinates with agency Pharmacy staff, Laboratory services, and Drug Representatives to ensure timely delivery of medications.
  • Collaborate with interdisciplinary team when necessary to ensure continuity of care.
  • Receive guidance and direction from supervisors.
  • Arrives/Reports to work on time and ready to work.
  • Performs other duties as assigned/required.
