SUMMARY

The LPN Evaluates clients for psychiatric illness, diagnosis, and initiate treatment. Provide medication management for assigned clients based upon approved protocols. Participates in Quality Improvement activities. Attending medical management staff meetings and mandatory staff training sessions.

EDUCATION and/or EXPERIENCE

This Licensed Practical Nurse position requires graduation from an accredited school of Practical Nursing and a valid Florida Nursing License. Experience working with individuals with severe and persistent mental illness is preferred.

CERTIFICATES, LICENSES, REGISTRATIONS

Must be licensed as an LPN in the State of Florida.

Possession of a valid Florida driver's license, acceptable driving record, and proof of personal automobile insurance if the position requires you to drive company vehicle and/or use personal vehicle for business. Complete State of Florida mandatory level 2 background screening prior to start of employment if direct contact with children/adolescents, vulnerable adults, or the developmentally disabled may be involved. Complete Mandatory training, CPR\First Aid and HIV/AIDS training during the first six (6) months of employment and updated if required.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides case management for an assigned group of clients, conducts comprehensive assessments, completes nursing assessments, consults with community agencies and families to maintain coordination in the treatment process, documents services, participates in daily staff meetings, participates in all services provided by the FACT Team.
  • Takes a lead role with the FACT Nursing Staff.
  • Takes a lead role on the Team for medical resourcing and coordination in the community.
  • Provides on-call crisis intervention on a rotating monthly schedule.
  • Takes the lead role and participates in medication administration and medical services.
  • Arranges and coordinates lab work.
  • Coordinates scheduling.
  • Administers injections.
  • Manages medications and samples.
  • Provides general nursing services in the clinic and in the field such as wound care and medical evaluations.
  • Obtains Prior Authorizations (PA) through insurance companies in a timely manner to ensure medical continuity of care.
  • Coordinates with agency Pharmacy staff, FACT Team Staff, Laboratory services, and Drug Representatives to ensure timely delivery of medications.
  • Manages biohazard waste control for the department.
  • Schedules psych appointments, makes referrals and coordinates with community medical providers.
