Case Specialist - FACT IL

QUALIFICATIONS:

The FACT IL Case Specialist needs to be able to make sound decisions independently on a daily basis, including but not limited to recognizing emergencies and life-threatening situations and responding appropriately.

Strong communication skills are essential and this individual must be able to interact appropriately with internal and external customers, including families, caregivers, community service providers, supervisory staff and other department professionals

OTHER QUALIFICATIONS:

Successful completion of Background, MVR and DFWP Screenings.

Each FACT IL team member contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness.

SUMMARY

The FACT IL Program is a multi-disciplinary, clinical team which assumes responsibility for directly providing needed treatment, rehabilitation and support services to adult persons with severe and persistent mental illness promoting recovery, empowerment, self-determination and decision-making for each FACT IL participant.

Monday through Friday (rotating on-call for after hours, weekends, and holidays)

EDUCATION/EXPERIENCE

  • Bachelor's degree in a behavioral science field, with preference for individuals who have completed a Targeted Case Management Certification (TCM). Individual will also be expected to become peer certified within 1 year of hire

  • Previous experience working with adults with severe and persistent mental illness (i.e. schizophrenia, bipolar disorder, borderline personality disorder) is required

  • Each FACT IL team member contributes to the completeness and confidentiality of clinical records by ensuring documentation, paperwork and system entries meet internal and external guidelines for content, accuracy and timeliness

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Working knowledge of Microsoft Office Programs, email, and the use of the internet is required
  • With the client, translate assessment outcomes into functional domains, prioritize needs under each domain into immediate, short-term, and long-term categories, and identify appropriate case management interventions
  • Conducts mental health status assessments and may assist with biopsychosocial assessments as needed
  • Involve meaningful collateral sources/others in initial and on-going case management service planning as appropriate and required.
  • Use ongoing assessment and collaboration with the client and meaningful collateral sources/others to review and update the case management plan to reflect emerging needs and maintain relevance.
  • Develop professional networks in order to identify and maintain information about available community resources to meet client needs
  • Make formal and informal referrals to service providers identified in the case management plan
  • Provides treatment, rehabilitation, and support services in accordance with DCF Guidance 36 Linking, Advocating, Transitioning, & Recovery Support (LATRS) Florida Assertive Community Treatment (FACT), Tier 2
  • Assures the accuracy, completeness and confidentiality of clinical records
  • Advocates for individuals served by the program to ensure acquisition of necessary social services to include activities of daily living, including medical, dental, legal, housing, and entitlements and monitors and evaluates the effectiveness of the service and the person's satisfaction
  • Provides coordination and linkage to community-based services to improve stability using the social determinants of health
  • Provides assistance with activities of daily living, transportation, communication, social and interpersonal relationship skills
  • Provides education related to symptoms, medications, and mental health issues
  • Provides service coordination for an assigned group of individuals which includes monitoring process made towards goals and objective and developing, writing, implementing, evaluating and revising the recovery plan in partnership with the individual and other providers

There are no supervisory responsibilities for this position.
