What You’ll Do

• Provide case management services and electronically document all activities in compliance with agency, state, and Medicaid standards
• Coordinate care by linking clients to internal programs and community resources
• Advocate for clients and ensure access to needed services
• Work closely with individuals, families, and natural support systems in home and community settings
• Monitor service delivery to ensure needs are met and goals are achieved
• Maintain accurate, timely documentation and ensure compliance with all regulations
• Collaborate with team members and contribute to program and agency improvements

What You Bring

• Bachelor’s degree in Psychology, Social Work, Health Education, or related human services field

AND one of the following:
• 1 year of experience working with adults with serious mental illness and/or children with serious emotional disturbances
OR
• 3 years of experience working with children with serious emotional disturbances
OR
• 2 years of experience working with adults with serious mental illness

• Certification or certification-eligible by Medicaid as a Case Manager

Preferred

• Experience in outpatient mental health or case management settings
• Strong advocacy and care coordination skills
• Excellent written and verbal communication skills
• Experience with electronic health records (EHR) systems

Additional Requirements
