Make a Difference as a Community Case Manager
Location: Community-Based | Pay: $20.35/hr | Full-Time

We are currently seeking a Community Case Manager to support individuals experiencing homelessness and serious mental illness. You’ll be out in the community—meeting people where they are—and helping them access the services, care, and stability they deserve.

What You’ll Do:

As a Community Case Manager, you'll be an advocate, connector, and problem solver. Your responsibilities include:

• Developing and implementing personalized service plans.
• Coordinating and tracking services using our electronic health record system.
• Building trust with individuals and families in their natural environments.
• Connecting people to resources such as housing, treatment, employment, and healthcare.
• Ensuring compliance with Medicaid, state, and agency policies.
• Collaborating with a dedicated team to improve lives every day.

What You Bring:

• Bachelor’s degree in Social Work, Psychology, Public Health, or a related field.
• At least 1 year of direct experience working with adults with serious mental illness or children with serious emotional disturbances.
• Strong communication skills and a compassionate, community-first mindset.
• Comfort working in unstructured, non-traditional environments.
• Must pass a Level II Background Screening, have a valid Driver’s License, and be eligible for Case Management Certification through the Florida Certification Board.
