QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION / EXPERIENCE:
Master's degree from an accredited college or university emphasizing in psychology, social work, health education or related human services. Minimum of three (3) years' experience in leadership or a supervisory position. Minimum of three (3) years of full-time experience working with individuals and/or children who have serious mental illness / serious emotional disturbances and/or substance use disorder.
CERTIFICATES / LICENSES:
Certification through the Florida Certification Board as a Behavioral Health Case Management Supervisor. Credentialed with Medicaid as a Targeted Case Manager Supervisor. Must have or obtain a SOAR certification within 6 months of employment.
SPECIAL SKILLS:
Experience in providing case management and advocacy services to children and adults in an outpatient mental health setting. Excellent written, verbal and computer skills necessary. Demonstrated leadership skills, including proven success in a supervisory or leadership role, can assist and guide in the management of caseload and documentation requirements with subordinates.
SUMMARY:
Responsible for ensuring continuity of care through advocating, planning, linking, coordinating, and monitoring of services and resources to help improve the quality of life for individuals served.? Work closely with Case Management Team Lead on assigned responsibilities beyond Case Management duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
SUPERVISORY RESPONSIBILITIES:
Will monitor caseloads, staff evaluations, peer review and compliance issues through consistent monitoring of status of service delivery events in the agency's electronic health record. Provide training for case managers. Conduct weekly case management team meetings. Meet with case managers individually to provide supervision. Attend meetings with outside agencies to ensure continuity of services for case management individuals.
