EDUCATION and/or EXPERIENCE:
Typically, these skills are the result of completion of a combination of education and experience equivalent to completion of a bachelor's degree in a social science such as psychology, social work, sociology.
SUMMARY:
As a part of a multidisciplinary
treatment team, the Behavioral Health Care Coordinator role is to participate
in the development of a comprehensive treatment plan, coordinate both medical,
behavioral health, and specialty care visits, track and monitor progress and
makes appropriate referral and linkages in ancillary services and community
resources. The Care Coordinator would provide services in a supportive,
confidential, and culturally competent manner to reduce stigma associated with
accessing behavioral health services.

EXPERIENCE/KNOWLEDGE:
- Knowledge of support services that serve Nassau County community.
- Working knowledge of Agency policies and procedures.
- Strong clerical/administrative skills including personal computer skills (i.e., Microsoft Office software).
- Ability to make routine decisions and determine proper action based on experience within the limits of policy and procedures.
- Effective organizational skills and the ability to handle multiple activities with changing priorities simultaneously.
- Good oral/written communication skills, effective listening, and interpersonal and good oral presentation skills with small and large groups.
- Ability to work effectively with people of diverse cultures, ages, and economic backgrounds.
ESSENTIAL DUTIES AND
RESPONSIBILITES:
- Coordinate
and monitor behavioral health visits including initial assessment appointment
after individual is released from their detox or inpatient setting.
Appointments should be made within 7 days of release.
- Responds
to referrals within 72 hours by coordinating with
inpatient and detox staff.
- Complete
on-site visits with referral individuals at their place of residence or partner
location, to establish rapport.
- Utilizes
electronic medical records and Substance Tracker to document and track all care
coordination services and referrals within 24 hours.
- In
coordination with therapist and medication management teams, develop
comprehensive treatment support plans for the identified individual on a weekly
basis.
- Complete
necessary documentation to facilitate referrals to collaborate with specialty
mental health providers to ensure continuity care.
- Develop,
compile, and disseminate culturally relevant resources/referral directory in
the broader community.
- Assist
individuals served in decreasing system barriers and navigating the social
services network. This may include transportation to the initial visit after
release.
- Advocates
for and links the individual with identified needed services and resources
prior to or after release. Coordinates all services ensuring the individual's
needs are being met toward completion of service plan goals.
- Weekly
reviews and updates individual's medical record and documents care coordination
activities according to Agency, State, Medicaid and other accreditation
standards and policy and procedures and enters information into the electronic
health record.
- Maintains
productivity standards regarding provision of direct services to individuals as
established by the agency.
- Participates
as a team member and contributes to the enhancement of the department and the
agency. Works with all individuals, employees, and supervisors in a cooperative
manner and treats them with dignity and respect.
