Pay: $18.50/hour

As a Care Coordinator, you'll work on a multidisciplinary team to support individuals navigating behavioral health, medical, and community-based services. What You'll Need to Succeed:

• Passion for supporting people from diverse backgrounds

• Knowledge of local support services (Nassau County preferred)

• Strong organization & multitasking skills

• Effective communication and teamwork abilities

• Clerical and computer skills (Microsoft Office proficiency)

• Ability to take initiative and problem-solve with care and compassion

Education & Experience:
Bachelor's degree in a social science field preferred (Psychology, Social Work, Sociology, etc.) or equivalent experience.

What You'll Be Doing:

• Coordinate behavioral health services post-detox/inpatient care

• Conduct outreach and establish rapport with individuals

• Build and manage care plans in collaboration with therapists and providers

• Help individuals access vital resources-transportation, referrals, support services, and more

• Advocate for client needs and remove barriers to care

• Maintain accurate and timely records using our electronic health systems

• Be a trusted guide on each client's journey toward stability and success
